SmartCare aims to define a common set of standard functional specifications for an open ICT platform enabling the delivery of integrated care to older European citizens. A total of 23 regions and their key stakeholders will define a comprehensive set of integration building blocks around the challenges of data-sharing, coordination and communication. BeyondSilos is an EU co-funded pilot project that allows providers in seven European regions to implement and pilot integrated social and health care services. Up to 10,000 people in Northern Ireland, Bulgaria, Spain, Italy, Portugal and Germany are expected to receive improved services by 2015. A key role in the services will be played by different ICT tools (from home health monitoring to shared data records) that will be utilised to enable better cooperation between care professionals, family members and those receiving care. MANAGE-CARE is currently the largest public health project in the chronic care sector funded from the European commission. It consists of 37 partners covering nearly the whole European Union and focuses onto the development of chronic care management standards and guidance in Europe.
